Texas EPI-2 Infectious Disease Report Form

Texas providers who suspect or confirm a notifiable condition report it to the local or regional health department, and the Department of State Health Services' EPI-2 Infectious Disease Report is the general form for doing so. It also covers outbreaks, exotic diseases and unusual groups of illness of public health concern. A health department epidemiologist may contact the reporting provider to investigate further.

Who completes it

The health care provider who diagnoses or suspects the condition completes the whole form except the health department box, which records whether the case is confirmed, probable, suspected, dropped or a duplicate.

Submitting and payment

How to submit it
Fax or send it to your local or regional health department; cases may also be reported to DSHS by phone as needed. Timeframes on the Texas Notifiable Conditions List are immediately, within 1 working day or within 1 week, and urgent conditions are phoned in.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Where do I send the Texas EPI-2?

To your local or regional health department, which handles reports for the patient's area. DSHS posts contact information for each local and regional health department on its notifiable conditions pages. As needed, cases may also be reported directly to DSHS by phone.

Do I have to wait for lab confirmation to file an EPI-2?

No. The form is for suspected cases as well as cases, and its general instructions ask for any outbreak, exotic disease or unusual group expression of disease of public health concern to be reported by the most expeditious means available.
