Vermont Reportable Disease Case Report (Online)

Vermont requires health care providers, nurses, physician assistants, infection preventionists and others to report suspected or confirmed reportable diseases to the Department of Health. This online case report form is for conditions reportable within 24 hours. It asks for the condition, lab testing, the reporter and the reporting facility, and goes to the Department's Infectious Disease Epidemiology staff.

Who completes it

The person reporting completes the whole form and names the reporting facility and its provider; examples of reporting facility providers include a school nurse, director of nursing or medical director.

Submitting and payment

How to submit it
Submit the online form, or fax the report, for conditions reportable within 24 hours. Immediately reportable conditions such as measles, hepatitis A, tuberculosis disease and meningococcal disease go by phone to the 24/7 line instead.

Good to know

  • Do not use the form for outbreaks or routine Lyme disease cases; outbreaks of COVID and flu-like illness remain reportable by other means.
  • Vermont's updated Reportable and Communicable Diseases Rule took effect June 10, 2026; individual COVID cases and test results are no longer reportable.
  • Latent tuberculosis infection is reportable within 24 hours by telephone rather than on the online form.
  • Labs report through electronic laboratory reporting, and EHRs can send electronic case reports automatically in near real time.
