Vermont Rabies Postexposure Prophylaxis Report

Vermont makes rabies postexposure prophylaxis in humans reportable to the Department of Health within 24 hours, and this one-page fillable form is how providers and facilities report it. It records who started PEP, the vaccine and immune globulin dates, the exposure and the animal involved. It goes to the Department's Infectious Disease Epidemiology Program, which also takes questions about it.

Who completes it

The health care provider or facility giving rabies PEP completes the whole form; if the reporter is not the treating provider, the provider's name goes in its own field. A box at the top is for VDH use only.

Submitting and payment

How to submit it
Fax it, or scan and email it, to the Vermont Department of Health within 24 hours. Human or animal rabies cases themselves are immediately reportable by phone.

  • Animal disposition is recorded as 10-day confinement, euthanized and tested, at large or unavailable, or unknown, with a free-text description of the exposure scenario.
  • The form asks whether a Town Health Officer has been notified, which is required for animal bites.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
