Massachusetts 51A Report: DCF Child Abuse Form

Massachusetts mandated reporters who have reasonable cause to believe a child under 18 is suffering abuse or neglect must first report by phone to DCF, then send this written 51A report within 48 hours. It documents the children, caregivers, the nature and extent of the injury or neglect, and related concerns, and goes to the DCF area office serving the child's city or town.

Who completes it

The mandated reporter completes every section, including the reporter block that asks whether the caregiver has been told of the report, and signs it. Nurses, physicians and other professionals whose work brings them into contact with children are mandated reporters; the family completes no part.

Submitting and payment

How to submit it
After the phone report (to the area office on weekdays, or the Child Abuse Emergency Line nights, weekends and holidays), submit the written report within 48 hours online through DCF Partner Services on the Virtual Gateway with a MyMassGov account, or by fax or mail to the area office.
Who pays
No fee or billing code applies; the written report is a legal duty of mandated reporters under M.G.L. c.119, s.51A.

Good to know

  • The written report never replaces the call: DCF requires the oral report first, then the written 51A within 48 hours.
  • First-time online filers must create or update a MyMassGov account to reach DCF Partner Services, and DCF posts a training video on submitting the written follow-up report.
  • Related-concern checkboxes include substance use, substance-exposed newborn, neonatal abstinence syndrome, domestic violence, human trafficking, mental or behavioral health and housing instability, with space to explain each.
  • The narrative asks the reporter to cite the source of any information not observed firsthand, and to note any concerns for social worker safety.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

When is the Massachusetts 51A written report due?

Within 48 hours of the oral report. Mandated reporters must call the DCF area office for the child's city or town immediately, or the Child Abuse Emergency Line after hours, then complete and send the written 51A to the same area office within 48 hours, online, by fax or by mail.

Can I file the 51A online?

Yes. After calling, mandated reporters can submit the written report and supporting documentation through DCF Partner Services on the Virtual Gateway. You need a MyMassGov account, and first-time filers use a different link from returning filers. Filing online does not replace the required phone call.

What does the 51A ask about the incident?

The nature and extent of the injury, abuse, maltreatment or neglect, any prior evidence, how the reporter became aware of it, incident dates, a Pedikit number if applicable, actions already taken to treat or shelter the child, and the name, contact details and relationship of the person responsible, if known.
