Massachusetts DPH Online Case Report Forms

Massachusetts DPH is moving provider case reports from paper to an online reporting system. Clinicians who diagnose gonorrhea, syphilis of any stage, HIV/AIDS, varicella, alpha-gal syndrome or hepatitis B in pregnancy, or find a positive TB test or suspect active TB, submit the case through a web form instead of paper. Facilities use the same system for gastrointestinal, respiratory and varicella clusters.

Who completes it

The diagnosing or treating provider, or staff on their behalf, completes the online form for the condition. Facility staff complete the cluster forms for outbreaks in healthcare settings, child care or schools; there is no patient section.

Submitting and payment

How to submit it
Open the condition's form from the DPH Infectious Disease Case Report Forms page and submit it online. If no form fits, or you are unsure how to report, call the Bureau of Infectious Disease and Laboratory Sciences; DPH lists a support mailbox for technical problems.
Who pays
Neither the DPH form list nor the online system lists a fee or billing code for submitting a case report.

Good to know

  • Paper chlamydia case reports are no longer required, and breakthrough COVID-19 cases resulting in hospitalization or death no longer need to be reported.
  • Some reports stay on paper: the Infant Case Report Form, the chancroid, LGV, granuloma inguinale and PID form, and the vaping-associated lung injury forms.
  • The HIV/AIDS form covers new diagnoses and previously diagnosed patients who are new to your facility, and DPH links instructions for the adult case report.
  • A respiratory cluster means one positive influenza test, or three or more people with fever of 100F or higher plus cough or sore throat; submit one form per cluster.

Get the note ready with Vero

This form is completed online with Massachusetts Department of Public Health, Bureau of Infectious Disease and Laboratory Sciences, so there is no PDF to fill. Vero Scribe writes the visit note it draws on, so the details are ready when you complete it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Which TB results go on the Massachusetts online form?

Any positive TB test, either an IGRA or a tuberculin skin test, and any person suspected of having active pulmonary or extrapulmonary TB. Both are reported on the Tuberculosis Disease and Infection (LTBI) Reporting Form in the online system, so latent infection and suspected disease use the same form.

Do shingles cases need to be reported in Massachusetts?

No. The varicella case report covers chickenpox, and DPH notes that shingles cases do not need to be reported. Facilities report three or more varicella cases within 42 days as a cluster, one form per cluster, with a new form only once 42 days have passed since the last case.

Do I still send a paper chlamydia report?

No. DPH states that paper case reports of chlamydia are no longer required. Gonorrhea and syphilis are reported through their online forms, while chancroid, lymphogranuloma venereum, granuloma inguinale and PID of unknown agent still use a paper case report form posted as a PDF and Word file.
