DC Notifiable Disease and Condition Case Report (DCRC)

DC Health requires health care providers to report notifiable diseases and conditions under DCMR Chapter 22-B2, and this electronic form is how most of them reach its Division of Epidemiology-Disease Surveillance and Investigation (DE-DSI). It covers every notifiable condition except HIV, STDs, viral hepatitis B and C, and tuberculosis, which go to HAHSTA. The form sits in the DC Reporting and Surveillance Center (DCRC).

Who completes it

The health care provider who diagnoses or suspects the notifiable condition completes the whole online report. It goes to DE-DSI rather than to HAHSTA.

Submitting and payment

How to submit it
Complete and submit the Notifiable Disease and Condition Case Report Form online through DCRC. Condition-by-condition timeframes are on DC Health's Notifiable Diseases and Conditions Resources page.
Who pays
DC Health lists no fee or payment for the report, which DCMR Chapter 22-B2 requires of all health care providers.

Good to know

  • HIV, syphilis, gonorrhea, chlamydia and viral hepatitis B and C go on the HAHSTA Case Report Form, and tuberculosis on the TB Case Report Form, both reached through the same DCRC link.
  • Pregnant patients with HIV, hepatitis B or syphilis are also reported on the separate Notifiable Disease and Pregnancy Report Form sent to HAHSTA.
  • The electronic DCRC form replaced DC's paper Communicable Disease Case Report Form, which was previously used to report cases to DE-DSI.
  • Animal bites and possible rabies exposures have their own Animal Bite Report Form in DCRC, and Zika testing uses the DCRC Zika Test Request and Reporting Form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Which diseases are reported on the DC notifiable disease form?

Every notifiable disease or condition on DC Health's list except HIV, sexually transmitted diseases, viral hepatitis B and C, and tuberculosis, which go to HAHSTA on its own forms. The full list of notifiable conditions and their reporting timeframes is posted on DC Health's Notifiable Diseases and Conditions Resources page.

Is there still a paper case report form in DC?

No. DC Health states that the electronic DCRC Notifiable Disease and Condition Case Report Form replaces the Communicable Disease Case Report Form, the paper form previously used to report cases to DE-DSI. Providers now complete the report online in DCRC, the same system used for HAHSTA, TB and animal bite reports.

Who must report notifiable diseases in DC?

DC Health states that all health care providers are required by law to report certain diseases and conditions under DCMR Chapter 22-B2. Laboratories in the District have their own duties, including reporting all positive acid-fast bacilli results, whether found by smear, culture or another method.
