Washington DOH 150-002 Adult HIV/AIDS Case Report

AIDS and HIV infection are reportable to local health authorities in Washington under WAC 246-101, within 3 working days and without patient consent. DOH 150-002 is the state's adult case report, the Washington version of the CDC adult form. It collects identifiers, diagnosis facility and provider, HIV diagnostic and care tests, patient history, opportunistic illnesses and treatment history.

Who completes it

The health care provider who diagnoses or treats HIV completes pages 1 and 2, leaving the 'For State Health Department Use Only' and local fields blank. Page 3 holds footnotes and code lists.

Submitting and payment

How to submit it
Mail the completed form to the Washington DOH Office of Infectious Disease Assessment Unit, or report through the local health jurisdiction, within 3 working days.

Good to know

  • At least two antibody tests must be indicated for an HIV diagnosis; if lab tests are not documented, the form asks whether a clinical care provider confirmed the diagnosis.
  • If a case progresses to AIDS, the form asks the provider to notify the health department; patient identifiers are not sent to CDC.
  • Washington law requires health officers and providers to offer partner notification assistance to people with HIV (WAC 246-100-209), and DOH field services can help.
  • Children are reported on the CDC 50.42B Pediatric HIV Confidential Case Report rather than this adult form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Do I need patient consent to report HIV in Washington?

No. Page 3 of the form states that HIV/AIDS cases are reportable within 3 working days under WAC 246-101 and that reporting does not require patient consent. Case reports are kept as confidential records, and several state laws protect patient identifying information.

What does Washington's HIV case report ask about treatment?

The history section asks whether the patient has ever taken antiretrovirals and lists HIV treatment, PrEP, PEP, PCP prophylaxis and other ARVs with start dates, current use and last use. It also asks about methamphetamine use, referrals to HIV medical and case management services, and current pregnancy.
