Washington DSHS APS Online Report

Washington's Department of Social and Health Services takes reports of suspected abuse, neglect, abandonment or financial exploitation of vulnerable adults through an online form. It covers adults living in the community and in licensed long-term care settings such as nursing homes, adult family homes and assisted living. Adult Protective Services and/or Residential Care Services screen every report.

Who completes it

Any concerned person or mandated reporter, including health care providers, completes the whole online report with the adult's name and contact details, the concerns, and their own name and contact. Proof is not required, and reporters need not give their name.

Submitting and payment

How to submit it
DSHS encourages online reports, which are available 24 hours a day and return a confirmation number; reports can also be made by phone. Call 911 if the person is in immediate danger.

Good to know

  • Employees making a mandated report about a licensed facility or certified supported living agency should use the separate Residential Care Services online report instead.
  • Unless there is a court action, law enforcement is involved or the reporter agrees, the reporter's identity is kept confidential.
  • Good-faith reporters cannot be held liable for damages resulting from the report, while intentionally false or malicious reports are a misdemeanor under RCW 74.34.053.

Get the note ready with Vero

This form is completed online with Washington State Department of Social and Health Services (DSHS), Aging and Long-Term Support Administration, so there is no PDF to fill. Vero Scribe writes the visit note it draws on, so the details are ready when you complete it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Do I need proof to report vulnerable adult abuse in Washington?

No. DSHS says you do not need proof to report suspected abuse, and you do not need to give your name. Giving your name and a way to reach you lets the investigator ask follow-up questions, and your identity stays confidential except in court action, law enforcement involvement or with your consent.

Which form do facility staff use in Washington?

Employees making a mandated report for a licensed facility or a certified supported living agency are directed to the Residential Care Services online report rather than the general APS form. Both routes are screened by Adult Protective Services and/or Residential Care Services.
