Colorado Confidential Syphilis Case Reporting Form

Colorado providers use this 1-page form to report syphilis cases to CDPHE's STI/HIV/Viral Hepatitis Branch. It records the patient, partners in the last 3, 6 and 12 months, a facility contact, HIV and pregnancy testing, nontreponemal and treponemal results with titers, stage, neurologic, ocular or otic involvement, and treatment dates. It can only be sent by email or fax.

Who completes it

The diagnosing or treating provider or facility completes the whole form and names a facility contact person for STD reporting.

Submitting and payment

How to submit it
Fax or email the form to CDPHE's confidential STI reporting line or address printed on it. STI treatment information can also be reported through the state's Reportal.
Who pays
There is no fee to file, and the form has no billing section.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
