Colorado Animal Bite or Exposure Report Form

Colorado uses this 3-page report from its Zoonoses and One Health program to follow up human animal bites and exposures for rabies. It records the animal (domestic or wild), its status and rabies vaccination, the exposure date, place and circumstances, the animal control office, the victim, the owner, the treating physician, and whether rabies PEP was recommended or started.

Who completes it

The health care provider treating the exposed person, or another reporter, completes all three pages; required fields are starred.

Submitting and payment

How to submit it
Faxing this form is currently the only way providers can report animal bites in Colorado. Return it to the local public health agency or animal control, or fax it to CDPHE if the local contact is unknown; on weekends and holidays, call the local public health agency.
Who pays
There is no fee to file, and the form has no billing section.

Good to know

  • Animal status options are owned, stray or feral, alive and in quarantine, waiting to be tested, location unknown, euthanized or unknown.
  • If the animal has not been quarantined, the form asks why not, and it asks how the animal was acting and what the victim was doing.
  • The current version is marked Updated August 2026 and is published by CDPHE's Communicable Disease Branch.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can Colorado providers report an animal bite online?

No. CDPHE states that faxing the animal bite or exposure report form is currently the only method available to providers for reporting animal bites. On a weekend or holiday, call the local public health agency instead.

Does the Colorado bite form ask about rabies PEP?

Yes. The treatment section asks whether the local health department has recommended rabies post-exposure prophylaxis and whether PEP has started. The form also records the treating physician's name, practice and phone.

What animal details does the Colorado bite report need?

The form asks whether the animal was domestic (dog, cat, livestock, wolf hybrid) or wild (bat, fox, raccoon, skunk, rodent), its status, breed, color and sex, rabies vaccination and date, and whether animal control was notified.
