Nevada Confidential Morbidity Report Form

Nevada uses one form for all legally reportable diseases, from animal bites and STDs to HIV, hepatitis, TB and COVID-19. NAC Chapter 441A requires physicians, nurses, labs, facilities and others to report cases, suspect cases and carriers within 24 hours to the local health authority. The four-page packet (updated September 2022) includes the disease list, instructions and contacts.

Who completes it

The attending physician or the person reporting for the facility completes the form, and a checkbox marks forms completed by the local health department. Attaching a patient face sheet is an acceptable way to give demographics.

Submitting and payment

How to submit it
Fax the form to the patient's local health authority: Southern Nevada Health District (with separate STD/HIV and TB lines), Washoe County, Carson City Health and Human Services for Carson, Lyon and Douglas counties, or DPBH elsewhere. Diseases needing immediate investigation are also phoned in.
Who pays
No fee applies; failure to report is a misdemeanor and may bring an administrative fine for each violation.

Good to know

  • Diseases marked with an asterisk on the list must be reported immediately, those with a dagger must be reported when suspected, and some are reportable in Clark County only.
  • Attach lab results and list pending tests and the performing lab in comments. If STD treatment details are left blank, the health authority will contact you.
  • The last page covers HIV tests, viral load and genotype, syphilis stage, chlamydia and gonorrhea sites, STI treatment, partner treatment, TB and COVID-19.
  • The instructions cite 45 CFR 160.204(b): HIPAA does not limit the authority of public health agencies to collect disease reports.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
