Nevada APS Online Intake Report (ADSD)

Nevada's Aging and Disability Services Division takes reports of abuse, neglect, self-neglect, exploitation, isolation or abandonment of older and vulnerable adults through an online APS intake form or by phone. Medical professionals and hospital and home health employees are mandatory reporters and must report immediately, no later than 24 hours after they have reason to believe abuse occurred.

Who completes it

The reporter completes the whole online report. Any person may report, and mandatory reporters include medical professionals, social workers, hospital and home health staff, and employees of facilities caring for vulnerable adults.

Submitting and payment

How to submit it
Use the APS Online Intake Report Form (WellSky) or call APS, which has separate lines for Las Vegas/Clark County and the rest of the state. Do not email reports to ADSD, and contact police or EMS first if the person is in immediate danger.
Who pays
ADSD lists no fee for reporting.

  • APS refers suspected crimes to law enforcement for possible prosecution, and provides protective services only if the person is willing to accept them.
  • APS staff never call about medical records, benefits or prescriptions; hang up on suspicious calls and contact local law enforcement.

Frequently asked questions

What is the deadline for mandatory reporters in Nevada?

ADSD says mandatory reporters must report immediately after the event, but no later than 24 hours after they have reason to believe a vulnerable adult has been abused, neglected, exploited, isolated or abandoned. Use the online intake form or call APS; for immediate danger, contact police, sheriff or emergency medical services first.

Is a Nevada APS report confidential?

Yes. ADSD states that all information received as a result of a report is maintained as confidential. APS investigates, evaluates the situation, and arranges or refers services, and it refers suspected crimes to the appropriate law enforcement agency.
