Maine APS Online Reporting Form (Adult Abuse)

Maine Adult Protective Services takes reports of abuse, neglect or exploitation of incapacitated or dependent adults by phone or through an online reporting form on the state's AccessGov site. A clinician who suspects a patient who cannot protect themselves is being mistreated, neglected or exploited uses it to notify APS, which investigates and can petition for public guardianship when less restrictive alternatives fail.

Who completes it

Anyone may file, including mandated reporters such as clinicians with care responsibility for an incapacitated or dependent adult; the reporter completes the whole online report. There is no section for the adult or family.

Submitting and payment

How to submit it
Complete the online APS Reporting Form on me.accessgov.com, or call APS on its 24-hour toll-free line; the APS mandated reporter page tells mandated reporters to call. Emergencies go to 911.
Who pays
OADS lists no fee; reporting is a legal duty for mandated reporters under Maine law.

Good to know

  • A dependent adult includes nursing home and assisted living residents, people receiving disability services, and anyone wholly or partly dependent on others for care because of significant limitations (22 M.R.S. 3472).
  • An incapacitated adult is 18 or older and unable to receive and evaluate information or to make or communicate decisions, even with supports.
  • Mandated reporters must complete department-approved APS mandated reporter training at least once every four years under 22 M.R.S. 3477.
  • A supervisor may not impede or inhibit a mandated report, and a person making a good-faith report may not be sanctioned for it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Should mandated reporters use the online form or call?

Maine's APS mandated reporter page tells mandated reporters to call APS, which answers 24 hours a day, and to report immediately when they know or suspect abuse, neglect or exploitation. The online form is listed on the main APS page as a second way to report. Life-threatening situations go to 911 first.

What counts as exploitation for Maine APS?

OADS lists financial exploitation, such as wrongfully taking or using the adult's property or resources, financial frauds or scams, medication theft and sexual exploitation. Neglect includes a caregiver failing to provide care, food, shelter, clothing or supervision, and self-neglect by an adult who cannot meet their basic needs.

What if the patient is under 18?

Concerns about anyone under 18 go to the Office of Child and Family Services intake line, not APS. Some child-abuse mandated reporters, including medical professionals and hospital staff, can also file non-emergency reports online through the Katahdin Child Welfare System.
