Massachusetts PET and PET/CT Prior Authorization Form

Massachusetts health plans accept a standard one-page form for prior authorization of PET and PET/CT imaging. Ordering clinicians, usually in oncology, meet it when a plan or its radiology benefit manager requires approval before a scan. Developed by the Massachusetts Collaborative and adopted with the Division of Insurance Bulletin 2016-08 imaging forms, it asks for tumor type, radiotracer, biopsy confirmation, treatment history and the reason for the study.

Who completes it

The ordering clinician's office completes all five sections: member, ordering provider, facility, the exam request with CPT and ICD codes, and the cancer details in Section 5. The patient completes nothing.

Submitting and payment

How to submit it
Send the form to the patient's health plan or its radiology benefit manager, with any additional data relevant to the plan's medical necessity criteria.

Good to know

  • Choose the radiotracer: standard FDG PET or PET/CT, sodium 18F fluoride PET bone scan, or another tracer described on the form.
  • Give the reason for the study: initial staging, restaging or surveillance, or interim PET/CT for response-adapted therapy, plus whether metastatic disease is known.
  • Section 5 asks if the patient is on or has completed chemotherapy or radiotherapy, with completion dates, and whether recurrence or progression is suspected.
  • The posted version is April 2019 (version 1.0); companion standard forms cover CT/CTA/MRI/MRA and cardiac imaging.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What cancer details does the Massachusetts PET/CT form require?

Section 5 asks for the tumor type and date of diagnosis, whether a biopsy confirmed the cancer, prior treatment (none, surgery alone or other), current or completed chemotherapy and radiotherapy, known metastatic disease, suspected recurrence or progression, and prior imaging results with dates.

Can the Massachusetts PET form be used for a PET bone scan?

Yes. The radiotracer choices include a sodium 18F fluoride PET/CT bone scan alongside standard FDG PET or PET/CT imaging, and an Other option for a tracer described in the space provided. The ordering clinician also enters the CPT and ICD codes in Section 4.
