Massachusetts Standard Home Health Prior Authorization

Massachusetts introduced a standard form in 2025 for home health prior authorization and reauthorization requests to insured health plans. Home health agencies meet it at the start of care or when an authorization period ends, working from the ordering physician's orders. It covers homebound status, functional status, requested nursing, aide and therapy visits with codes and frequency, wound care, and goals for the authorization period.

Who completes it

The home health agency completes the form, naming the ordering MD, PCP and NPI in the requesting provider section and writing the clinical summary, goals and interventions for each discipline. Checkboxes record whether the MD and the patient agree.

Submitting and payment

How to submit it
Send the completed form to the patient's health plan. When more than one skilled discipline provides care, complete a separate page 2 for each discipline.

Good to know

  • Mark whether the request is initial or a reauthorization, give the start-of-care date, and state the anticipated or actual agency discharge date.
  • The service grid lists RN, LPN, home health aide hours and visits, PT, OT, ST and MSW, each with CPT or HCPCS code, dates, visits or units and frequency.
  • Homebound status needs a yes or no with the reason, and functional status is rated independent, requires assist or unable for dressing, bathing, toileting, ambulation, eating, transfers and grooming.
  • The current version is dated August 2025. A companion standard form covers initial SNF, rehab and LTACH admissions.

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
