Massachusetts Standard ABA Prior Authorization Form

Massachusetts insured health plans use a standard form for prior authorization of applied behavior analysis (ABA) for autism spectrum disorder. A licensed applied behavior analyst meets it when requesting an evaluation, initial services, continued services or an amended continuing request. It lists ABA CPT codes, units and service locations for an authorization period of up to six months, and goes to the plan with a treatment plan.

Who completes it

The licensed applied behavior analyst (LABA) rendering or supervising the autism services completes and signs the form. An evaluation request needs only Section 1; initial, continued and amended requests need Sections 1 and 2.

Submitting and payment

How to submit it
Send the form with the treatment plan to the member's health plan or the behavioral health manager that handles its ABA authorizations. Requests for an initial evaluation should include the diagnostic report.

Good to know

  • Units are 15 minutes each. Enter either units per week or units for the whole authorization period, following the plan's policy, plus the planned service location.
  • Codes 0362T and 0373T, used when two or more technicians are needed for destructive behavior, require a clinical rationale in a separate attachment or the treatment plan.
  • Section 1 asks for a definitive ICD-10 F code and whether a licensed physician or psychologist completed the diagnostic evaluation, with the date.
  • Addendum 1 is a checklist of treatment plan features, from assessment results and goals to parent training, hours requested, care coordination, transition, discharge and crisis plans.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
