Massachusetts Non-OB Ultrasound Authorization Form

Massachusetts health plans accept a standard one-page authorization form for non-obstetric ultrasound studies. Ordering clinicians and imaging facilities meet it when a plan or its radiology benefit manager requires approval before an ultrasound. Developed by the Massachusetts Collaborative and required under Division of Insurance Bulletin 2017-04, it asks for the exam codes, the reason for the study, prior imaging and whether symptoms have changed since.

Who completes it

The ordering physician's office or the facility completes the form; it asks who is making the request (ordering physician, facility or other) and the signer's title, such as MD, RN, LPN, PA or NP. The patient completes nothing.

Submitting and payment

How to submit it
Send it to the patient's health plan or the radiology benefit manager that reviews its imaging, attaching any additional data relevant to the plan's medical necessity criteria.

Good to know

  • Section 4 asks the type of most recent documented contact with a physician, such as consultation, office visit, email or phone call, along with first and latest visit dates.
  • Prior imaging questions cover ultrasound, CT, CTA, MRI, MRA and x-ray, how recently the last study was done, and whether signs or exam findings have developed or worsened since.
  • The posted version is April 2019 (version 1.0). Bulletin 2017-04 required it along with the standard hepatitis C and Synagis forms.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Who can sign the Massachusetts non-OB ultrasound authorization form?

The form asks who is making the request: the ordering physician, the facility or someone else. The signer prints their name and checks a title, MD, RN, LPN, PA, NP or other, then signs and dates it, so clinical staff can submit it for the ordering physician or facility.

What prior imaging details does the Massachusetts ultrasound form ask for?

It asks whether there was prior imaging for the condition (ultrasound, CT, CTA, MRI, MRA or x-ray), when the most recent study was done, from under a week to over a year ago, and whether new or worsening signs, symptoms or exam findings have appeared since that study.
