Massachusetts Hepatitis C Medication PA Form

Massachusetts insured health plans use a standard form for prior authorization of hepatitis C antiviral therapy. Prescribers meet it when starting or continuing a regimen such as Epclusa, Mavyret, Harvoni or Zepatier for a plan member. Updated with Division of Insurance Bulletin 2024-03, it captures genotype, fibrosis stage, HCV RNA levels and prior treatment response, and includes a step therapy exception section.

Who completes it

The prescribing clinician completes Sections A to F, from destination plan and patient details to clinical information and step therapy exceptions. The clinician or an authorized representative signs in Section C.

Submitting and payment

How to submit it
Fax or send the form to the patient's health plan or prescription plan named in Section A. Some plans might not accept this form for Medicare or Medicaid requests.

Good to know

  • Section E asks for genotype, fibrosis stage F0 to F4 (and whether F4 is compensated), and the assessment method and result, such as FibroScan, FibroSure, APRI or FIB-4.
  • Record baseline HCV RNA with the lab date; continuation requests also need the week 8 level. HIV coinfection, liver transplant status and GFR range are also asked.
  • The form asks whether a gastroenterologist, infectious disease specialist or hepatologist prescribes or consults, and Zepatier requests ask about NS5A polymorphism testing for genotype 1a.
  • Checking the expedited box is an attestation that the request meets the carrier's definition and criteria for an urgent review.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
