Maryland Uniform Dental Consultation Referral Form

Maryland requires a uniform referral form when a primary or requesting dentist refers a dental HMO or plan member to a specialist dentist for consultation, under COMAR 31.10.12. General dentists meet it whenever a patient needs a specialist's consultation or procedure under plan rules. The form records the reason for referral, history and test results, the services desired and how long the referral stays valid.

Who completes it

The primary or requesting dentist completes the patient, carrier, referring dentist, specialist and referral sections and signs. A second, authorizing signature is added only when required.

Submitting and payment

How to submit it
Distribute the copies as printed on the form: white to the carrier, yellow to the primary or requesting dentist, pink to the consultant or facility, and goldenrod to the patient, following the carrier's instructions.
Who pays
Referral certification is not a guarantee of payment; benefits depend on the member's eligibility on the date of service and the plan's other contract provisions.

Good to know

  • Mark the services desired, initial consultation only, consultation with specific procedures, or other, and the place of service: office, hospital or other.
  • Mark missing teeth with an X on the teeth diagram, and give a brief history, diagnosis and test results to support the referral.
  • Enter the authorization number if the carrier requires one and the date the referral is valid until, following the carrier's instructions.
  • The dental form took effect January 1, 2005. A separate Maryland Uniform Consultation Referral Form, required by the same COMAR chapter, covers medical referrals and is published by carriers.

Frequently asked questions

When is the Maryland Uniform Dental Consultation Referral Form used?

When a primary or requesting dentist refers a member of a dental HMO or plan to a specialist dentist for consultation. COMAR 31.10.12 sets the uniform form, so Maryland carriers receive referrals in one layout, with the specialist's details, reason for referral and services desired.

How long is a Maryland dental referral valid?

The form has a Referral Is Valid Until date that the referring office fills in according to the carrier's instructions, next to any required authorization number. The form sets no fixed period itself, so the carrier or plan manual governs how long a referral lasts.
