Minnesota Uniform Rx Prior Authorization Form C-2.0

Minnesota law requires prescribers to use one uniform form for prescription drug formulary exceptions and requires all payers to accept it; the same form serves drug prior authorization requests. Prescribers meet it when a health plan, HMO or PBM requires approval or an exception and the request is not sent through electronic PA. Payers can reply on the form's Section G with their determination.

Who completes it

The prescriber, or someone authorized to act for them, completes Sections A to F as applicable: destination payer, patient, prescriber, drug, clinical information and pharmacy. The payer completes Section G, and payers posting the form may pre-fill Section A.

Submitting and payment

How to submit it
Send it by fax or other secure transmission to the patient's health plan, PBM or other payer, never to the patient's employer, the Minnesota Department of Health or the Administrative Uniformity Committee.

Good to know

  • Minnesota Statutes 62J.497 subd. 5 requires drug PA requests to be submitted and accepted electronically using the NCPDP SCRIPT standard (version 2013101) since January 1, 2016.
  • Complete Section F, the pharmacy information with NDC, when the request goes to the Minnesota Department of Human Services; DHS does not cover clinical drug trials.
  • For patients with carved-out prescription benefits, enter the prescription benefit cardholder ID in Section B instead of the health plan ID.
  • Version C-2.0, current since October 2015, supersedes earlier MDH forms; the version and revision date print in the lower right corner.

Frequently asked questions

Do all Minnesota payers have to accept the uniform drug PA form?

Yes for formulary exceptions: Minnesota Statutes 62J.497 subd. 4 requires providers to use the uniform form and payers to accept it. The same form serves prescription drug PA requests, which the law otherwise requires to be exchanged electronically through the NCPDP SCRIPT standard.

Does the Minnesota uniform form work for Minnesota Health Care Programs members?

Yes. The form's instructions tell prescribers to complete Section F, pharmacy information including the NDC, when submitting a drug PA request to the Minnesota Department of Human Services, and note that DHS does not cover clinical drug trials.
