New Hampshire Uniform Prescription Drug PA Form

New Hampshire requires commercial insurers and pharmacy benefit managers to use and accept this uniform form for prescription drug prior authorization requests. It was adopted under RSA 420-J:18 and the Insurance Department's Ins 2700 rules, where it appears as Appendix B. A prescriber meets it whenever a New Hampshire insurer or PBM requires approval for a medication.

Who completes it

The treating or prescribing provider completes Sections B through G (request type, patient, prescriber, medication, clinical information and rationale) and signs. Section A identifies the insurer or PBM and is prefilled by them.

Submitting and payment

How to submit it
Fax it or send it electronically to the insurer or pharmacy benefit manager named in Section A.

Good to know

  • Insurers may not require information beyond what the uniform form asks for, so the completed form plus its clinical sections should be sufficient for a decision.
  • The rules behind the form are Ins 2705, which has the Insurance Commissioner develop uniform prior authorization forms and criteria, and RSA 420-J:18.
  • The current version is dated 2017. Medical services use a separate uniform medical prior authorization form, Appendix A of the same rules.
