ND WSI UR Chiropractic Review Request SFN 59693

North Dakota Workforce Safety & Insurance allows chiropractic care without prior authorization only during an initial window of 10 visits or 60 days, whichever comes first. To continue, the treating chiropractor submits this UR Chiropractic Review Request listing the ICD-10 code for each treated area, the manipulation code, modalities, visits requested and completed, and the worker's current work status.

Who completes it

The treating chiropractor completes Sections 1 to 4, including NPI, facility and the person to notify with the decision. The injured worker does not complete or sign any part.

Submitting and payment

How to submit it
Fax the form with recent medical notes to WSI's Utilization Review Division, or submit the request in myWSI. WSI completes the review within 3 business days of receiving all required information.
Who pays
A denied chiropractic bill is not reviewed on this form but on the Medical Bill Appeal (M6), which WSI allows only in limited situations, such as when the claim was not filed at the time of service.

Good to know

  • The initial evaluation or re-evaluation counts toward the window, and only one initial window is allowed per claim, not per body part or diagnosis.
  • Up to 2 modalities per visit are allowed during a window; select none, 1 or 2 and list the CPT codes for the modalities.
  • A new evaluation without prior authorization is allowed after a lapse in care of more than 2 weeks.
  • A 2-week extension can be requested by phone before an approval expires, but not for the initial window.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When does chiropractic care need prior authorization from WSI?

After the initial window of 10 visits or 60 days of care, whichever occurs first. The initial evaluation counts toward the window, and only one initial window is allowed per claim. Further visits need a UR Chiropractic Review Request on SFN 59693 or the same request in myWSI.

Which manipulation codes does the WSI chiropractic form list?

Section 3 offers CPT 98940, 98941, 98942 and 98943 for the manipulation requested, plus the number of modalities per visit (none, 1 or 2) and their CPT codes. It also asks for the ICD-10 code for each treated area and the start and end dates of upcoming treatment.

What else does SFN 59693 ask about?

Whether all previously approved visits have been completed, the date of the most recent visit and number of visits completed, whether the patient is also receiving physical or occupational therapy, and whether the worker is on full duty, restricted duty or not working. Section 4 leaves room for additional information.
