ND WSI M11 Medication Prior Authorization Request

Prescribers treating a North Dakota Workforce Safety & Insurance claim use the M11 when a medication, dosage or dosing interval needs prior authorization, or when the patient needs a brand-name drug instead of its generic. The request names the drug, strength, dosage form and diagnosis, and the prescriber certifies the medication is medically necessary and not prescribed for convenience or solely at the patient's request.

Who completes it

The prescribing provider completes all four sections, including NPI and facility, and signs the medical necessity certification in Section 4. The injured worker does not sign.

Submitting and payment

How to submit it
Send the M11 with supporting medical notes to WSI's Medical Services Division, or submit the prior authorization request online in myWSI.

Good to know

  • WSI requires a trial of the generic equivalent before a brand-name request; document the adverse reaction or inadequate response with measurable, objective evidence in the attached notes.
  • A brand-name approval applies to the specific medication, strength and dosage requested and lasts a maximum of one year.
  • To continue a brand medication past one year, check Renewal and submit a new M11 each year.
  • For a dose or interval request, describe the reason for the request and the expected duration of need in Section 3.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When do I need a WSI M11 for a brand-name drug?

When the patient needs the brand product instead of its generic equivalent. WSI's policy requires a generic trial first, then an M11 showing objective medical evidence of an adverse reaction or an inadequate therapeutic response to the generic that the brand product does not cause or produce.

How long is a WSI brand medication approval valid?

WSI's brand-necessary medication policy says approval applies to the specific medication, strength and dosage and is valid for a maximum of one year. The provider must submit the M11 each year to renew it, checking Renewal rather than Initial on the form.

What does the prescriber certify on the M11?

That the medication is medically necessary for the patient's well-being, reasonable and necessary under accepted standards of medical practice for the condition, and not prescribed as a convenience to the patient or solely because the patient asked for it. The prescriber signs and dates that statement in Section 4.
