North Dakota SFN 481 Service Limits Authorization

North Dakota Medicaid uses SFN 481 when a provider asks to exceed service limits for chiropractic care, physical, occupational or speech therapy, medical nutrition therapy, community health worker or paramedicine services, behavioral health rehabilitative services or targeted case management. Providers meet it when a member reaches a program limit but needs more care. The Medical Services Division approves or denies visits or units for a date range.

Who completes it

The billing or servicing provider completes provider, recipient and request details, explains medical necessity, signs, and checks the certification that the request is accurate and that the current coverage policy was reviewed. State staff complete the State Use Only section.

Submitting and payment

How to submit it
Fax or mail the form with clinical documentation to the ND HHS Medical Services Division, as printed on the form. Contact information and an email address are required for a determination.

Good to know

  • Check only one area of request per form, and list ICD diagnosis codes and the required CPT or HCPCS codes with visits or units and start and end dates.
  • Requests covering services already delivered (retroactive services) are considered only when the form gives a good cause explanation.
  • The form asks for the date of the member's last service limit request alongside the new request.
  • The current version is SFN 481 (2-2026), fillable on the North Dakota eForms site. Procedures, devices and drugs use SFN 511 instead.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
