Kansas Medicaid Power Wheelchair Prior Authorization

Kansas Medicaid (KMAP) requires prior authorization for a power wheelchair, and this two-page request carries the case. It asks how long the doctor says the chair will be needed, how far the beneficiary can walk, how a manual wheelchair is used, hours of attendant care, whether the beneficiary can operate power controls independently, and whether the home allows access.

Who completes it

The DME provider completes and signs the request with its provider number and NPI, the HCPCS code, manufacturer and model. The prescriber supplies the signed prescription with medical necessity for any options and the clinical facts behind the answers.

Submitting and payment

How to submit it
The DME provider faxes the completed request to KMAP prior authorization; it is returned unprocessed if incomplete, and the PA is denied if not received within 15 working days.
Who pays
The approved reimbursement goes to the DME provider and includes assembly of the wheelchair and all its components; a rental includes all repairs or modifications.

Good to know

  • Required attachments: manufacturer retail pricing for the chair and options (or an invoice for a used in-stock rental), a signed prescription with medical necessity for options, and verification of school or work hours.
  • Item 12 requires documentation of why a manual wheelchair does not meet the beneficiary's needs, and item 14 asks how they have managed so far and the plan if no power chair is provided.
  • PA does not guarantee eligibility, is void if KMAP does not cover the service, and does not override Primary Care Network referral or program limitations.
  • Manual wheelchairs and special wheelchair seating devices have their own KMAP PA request forms. This form is revised 10.2017.

Frequently asked questions

Who submits the Kansas Medicaid power wheelchair PA request?

The DME provider completes, signs and faxes it to KMAP. The prescriber's part is the signed and dated prescription, including medical necessity for any wheelchair options, plus the clinical information behind answers such as walking distance, length of need and ability to operate power controls independently.

What documents go with a Kansas Medicaid power wheelchair request?

Manufacturer retail pricing including options, or an invoice when renting a used in-stock chair; a signed and dated prescription with medical necessity for options; written verification of school or work hours; and documentation explaining why a manual wheelchair does not meet the beneficiary's needs.

Is there a separate Kansas form for manual wheelchairs?

Yes. KMAP publishes a Manual Wheelchair PA Request and a Special Wheelchair Seating Device PA Request alongside this power wheelchair form. The request type should match the equipment ordered, and each form must be completed in its entirety to avoid being returned unprocessed.
