Kansas Medicaid Diabetes Testing Supplies Order Form

Kansas Medicaid (KMAP) uses this one-page form as the prescription and medical necessity statement for blood glucose testing supplies. The physician, ARNP or PA checks the diabetes ICD-10 code, orders a testing frequency and length of need, records insulin use, and explains why more frequent testing is needed and how the results will be used.

Who completes it

The prescribing physician, ARNP or PA completes and signs the whole form, adding the physician's Kansas Medicaid provider ID and NPI. The DME provider completes a separate prior authorization request with the HCPCS codes and modifiers.

Submitting and payment

How to submit it
The completed form is faxed to KMAP, and KMAP will not consider it without the DME provider's PA request form; the PA is denied if the request is not received within 15 working days.
Who pays
The form sets no fee; it supports the DME provider's prior authorization and Medicaid billing for the testing supplies.

  • The reason for testing more often can be fluctuating or uncontrolled blood sugar, hypoglycemia or another explained reason, and the prescriber states how the additional results will be used.
  • The prescriber confirms whether the patient has been seen and diabetes control evaluated within the last 6 months.
  • KMAP returns the form unprocessed if any part is left blank. The current version is revised 10/2015.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is the Kansas diabetes supplies form a prescription?

Yes. The form states that it suffices as a script for blood glucose testing supplies. KMAP will not consider it on its own, though: the DME provider must also submit a prior authorization request form with the appropriate HCPCS codes and modifiers.

Who can sign the Kansas Medicaid diabetes testing supplies form?

A physician, ARNP or PA. The form asks for the physician's printed name, Kansas Medicaid provider ID and Kansas NPI, then the signature and date. The prescriber also states whether the patient has been seen and diabetes control evaluated within the last 6 months.

What happens if the Kansas diabetes supplies form is incomplete?

KMAP returns it unprocessed if it is not completed in its entirety, and the prior authorization is denied if the request is not received within 15 working days. Every field matters, including testing frequency, length of need, insulin injections per day and the reason for more frequent testing.
