Ohio ODM 02912 CMN: Incontinence Items

Ohio Medicaid uses this one-page certificate of medical necessity to document why an individual needs incontinence garments and related supplies, which items, and how many per month. The supplier can fill in the identifying details and transcribe the certification, but only the prescriber can sign the attestation. It covers initial certifications, changes and renewals.

Who completes it

The supplier may complete the identifying information and transcribe the certification section, including diagnosis codes, items and quantities. The prescriber must complete the attestation, signing and dating it.

Submitting and payment

How to submit it
Return the signed certificate to the supplier, which keeps it for its records and prior authorization requests; a previous PA number goes on the form when one exists.

Good to know

  • The form notes that Ohio Medicaid cannot pay for items related to stress incontinence with no attributable physiological, psychological or physiopsychological cause.
  • For residents of a long-term care facility, providing or paying for equipment and supplies is the facility's responsibility, as the form notes beside the address.
  • Length of need is under 12 months, 12 months or lifetime; each item is listed with a HCPCS code or style and size and a monthly quantity.
  • The prescriber's signature attests the certification is true, correct and complete, and the form warns that false certification constitutes Medicaid fraud. Current revision: 7/2024.
