NC Medicaid-3075 Physician's Request for PDN

North Carolina Medicaid uses this two-page physician request to start or continue private duty nursing (PDN) for a beneficiary. It documents diagnosis, prognosis, hours requested, caregiver availability, ventilator, oxygen and tracheostomy needs, and a summary of the last certification period. The form is to be completed within 15 business days of the start of care date and submitted to NC Medicaid.

Who completes it

The attending physician prints their name, address and phone, then signs and dates the request. The PDN agency's provider number, NPI and taxonomy are recorded on the same form.

Submitting and payment

How to submit it
NCTracks prefers prior approval requests online through the NCTracks Provider Portal. When mailed or faxed, the 3075 goes as the top page with all attachments behind it.

Good to know

  • Item 27 asks for a summary of nursing documentation for the last certification period, and the form says not to copy the CMS-485 plan of care into it.
  • Technology questions cover ventilator dependence and daily hours, oxygen flow and hours, the saturation target, and whether the patient has a non-ventilator tracheostomy.
  • The PDN PA Referral Form (3061) and Medical Update form (3062) are attachments only; NCTracks warns they will not create a PA request on their own.
  • NC Medicaid also lists this form as DHB-3075 on its Private Duty Nursing forms page; the current printed version is NC Medicaid-3075, dated 3/2024.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
