NC DMA-0100 Physician Signature for Level of Care

NCTracks uses this one-page form to tie a physician's signature to a level-of-care request entered online for an NC Medicaid beneficiary. The physician verifies that they provided the information on the state approved level of care form and that it is accurate, and marks the level they assessed, from nursing facility or ventilator care to CAP or PACE.

Who completes it

The physician who assessed the beneficiary marks the level of care, fills in the recipient name and ID and the determination date, and signs. The receiving facility's name and the move date are added if known.

Submitting and payment

How to submit it
Send the signed form with the level-of-care prior approval request; NCTracks lists it as a prior approval attachment that does not create a request on its own. The form prints a fax line for submission.

Good to know

  • Levels listed are NF, NF Rehab, ventilator, specialty hospital rehab, extended care, CAP/DA intermediate or skilled, CAP/C skilled or hospital, and PACE.
  • It records the date the level-of-care determination was made and, when known, the receiving facility and the date of the move to that facility.
  • It pairs with the FL2 (NC Medicaid-372-124); NCTracks warns that attachments sent without a primary request may be delayed or not received at all.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What is NC form DMA-0100 used for?

It is the physician's signature page for a level-of-care request submitted through NCTracks. By signing, the physician verifies that they provided the information on the state approved level of care form for the beneficiary, that it is accurate, and which level of care they assessed as appropriate.

Does DMA-0100 create a prior approval request?

No. NCTracks lists it under prior approval attachments, which accompany a primary request and do not create one. NCTracks warns that an attachment sent without its request may be delayed or not received at all, so it should go with the level-of-care request it supports.
