Kentucky MAP 248: Disposable Supplies Certification

Kentucky Medicaid's home health program uses MAP 248 to certify that disposable medical supplies provided through a home health agency are essential to the recipient's medical needs. The agency lists each supply by HCPCS code, quantity and dates, and the physician, APRN or PA certifies the patient requires them. The agency keeps the signed form.

Who completes it

The home health agency fills in agency and recipient information and the supply lines. The physician, APRN or PA marks the anticipated duration of need and signs with NPI, address and date.

Submitting and payment

How to submit it
The signed form is returned to the home health agency, which keeps it on file.
Who pays
The form sets no fee for the practitioner; it supports the home health agency's Medicaid billing for the supplies.

Good to know

  • The certification must be signed and dated by the physician, APRN or PA every six months.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
