Vermont Medicaid General Pharmacy PA Request Form

Vermont Medicaid uses this general form for prescription drug prior authorization when no drug-specific form applies, and Optum administers the pharmacy benefit for the Department of Vermont Health Access. A prescriber meets it when a member's medication requires prior authorization. The form also covers drugs billed under the medical benefit, which need a HCPCS J-code and the administering provider's details.

Who completes it

The prescriber completes the whole two-page form, signs and dates it. It covers the prescribing physician, beneficiary, pharmacy and, for medical benefit drugs, the administering provider or facility.

Submitting and payment

How to submit it
Fax the completed, signed form to Optum, which runs the Vermont Medicaid pharmacy help desk.
Who pays
The prescriber certifies the request is medically necessary and supported in the records, and misrepresentation can lead to audit and recoupment.

Good to know

  • Incomplete requests are returned for more information, so complete every section before signing and dating.
  • List preferred drugs tried and failed with reason and date; Optum may ask for clinical notes if trials are not in the member's claims history.
  • DVHA directs EPSDT drug coverage requests to the General PA Form for most requests.
  • About 30 drug and class-specific forms exist, such as multiple sclerosis, hemophilia factor, growth stimulating agents and an MME safety checklist.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
