Georgia Medicaid CMN for Oxygen Equipment

Georgia Medicaid requires this one-page certificate before it authorizes home oxygen equipment. The physician records a respiratory or cardiac primary diagnosis, the stationary and portable systems ordered, liters per minute, hours per day and qualifying blood gas or saturation results. The oxygen supplier submits it, with the lab report attached, as part of its prior authorization request.

Who completes it

The physician completes the clinical sections and signs the certification of medical necessity, including the date of the face-to-face evaluation. The supplier's name, address and NPI also appear on the form.

Submitting and payment

How to submit it
The oxygen supplier submits the signed certificate with its prior authorization request to Georgia Medicaid; a copy of the laboratory report must be attached.
Who pays
The form sets no fee for the physician; it supports the supplier's prior authorization for oxygen equipment.

Good to know

  • The physician must have had a face-to-face evaluation within 10 months before the order, which the form defines as within 304 days of the order date.
  • For members 21 and over, an ABG PO2 above 60 mmHg or saturation above 89 percent requires a written clinical rationale for oxygen.
  • Select no more than one stationary system and one portable system. If portable oxygen is ordered, state its purpose; if the test was not on room air, explain why.
  • Stamps are not accepted for the signature or date. Enter the member's own Medicaid number, not the mother's. The current version was revised 4/1/2025.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
