Hartford LC-7137 Attending Physician Progress Report

The Hartford uses this two-page progress report to update an open group disability claim after the initial statement, LC-7135. The provider completes it from the most recent patient contact, giving current disabling diagnoses with ICD-10 codes, symptoms and exam findings, the projected full-time return-to-work date, treatment plan, medications, level of functionality and diagnostic tests.

Who completes it

The treating provider completes the whole form and signs it with specialty, EIN and license number. The Hartford sends it for each claim with the patient's name and claim ID.

Submitting and payment

How to submit it
Fax, email or mail the completed report to The Hartford using the details printed on the form, quoting the claim ID.
Who pays
The form states the patient is responsible for its completion without expense to The Hartford.

Good to know

  • Base every answer on the most recent patient contact or evaluation, and record whether that visit was in person or by telemedicine.
  • The functionality section uses the same continuous, frequent, occasional and never scale as the initial statement, with hours at one time and total hours for sitting, standing and walking.
  • For pregnancy, give the actual or estimated delivery date. Therapy, surgery with CPT codes, and chemotherapy or radiation dates go in the treatment plan.
  • The circulating version is LC-7137-13, dated 06/2021. The Hartford does not post it, and employer-hosted copies are for reference only.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How is LC-7137 different from LC-7135?

LC-7135 is the initial statement for a new claim, with onset, first day out of work and first treatment date. LC-7137 is the progress report for an open claim: it focuses on current diagnoses, the projected full-time return-to-work date and current functionality, based on the most recent patient contact.

Does The Hartford pay for the progress report?

No. The form states the patient is responsible for completing it without expense to the company. Any fee the practice charges for the progress report is therefore the patient's responsibility, as with the initial statement, LC-7135.
