FEGLI Form FE-8: Living Benefits Claim Form

Federal Employees' Group Life Insurance lets an enrollee who is terminally ill, with a life expectancy of nine months or less, receive Basic life insurance as a living benefit. FE-8 is the claim form for that election, and a physician must certify the terminal illness. Clinicians meet it when a federal employee or annuitant brings the mailed form for certification.

Who completes it

The enrollee completes the claim, and the enrollee's attending physician provides the medical certification of terminal illness that the living benefits rules require.

Submitting and payment

How to submit it
There are no electronic copies. The employee or annuitant calls OFEGLI, which mails the application form, and the completed form is returned to OFEGLI.

Good to know

  • OPM's page states that agencies cannot order the FE-8, so the patient, not the agency HR office, has to request it from OFEGLI.
  • Eligibility turns on life expectancy: the enrollee must be terminally ill with nine months or less to live, as certified by a physician.
  • The living benefit comes from the enrollee's FEGLI Basic life insurance, and FE-8 is the claim form OFEGLI uses for that election.
  • Because OFEGLI mails the form only on request, practices cannot download a blank copy; complete the copy the patient brings in and follow its printed instructions.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How do I get FEGLI Form FE-8?

OPM does not post FE-8 online. Employees and annuitants call the Office of Federal Employees' Group Life Insurance (OFEGLI), which mails them the application form. Agencies cannot order it, so a clinician asked to certify the illness works from the copy the patient received.

Who qualifies for FEGLI living benefits?

An enrollee who is terminally ill with a life expectancy of nine months or less can elect to receive Basic life insurance as a living benefit. The attending physician's certification of the terminal illness is the medical evidence that supports the claim.

What does the physician certify on FE-8?

The attending physician certifies that the enrollee is terminally ill, with a life expectancy of nine months or less. OPM's public page does not reproduce the form, so follow the instructions printed on the mailed copy for which part to complete and what to attach.
