The Standard PFML-7: Own Serious Health Condition Form

An employee covered by a private paid family and medical leave plan insured by The Standard who takes leave for their own condition receives an Own Serious Health Condition packet, and Form PFML-7 is its provider certification. The provider documents medical facts and the leave needed, including flare-ups. The Standard decides a complete claim within 5 days of receiving it.

Who completes it

The employee enters their name and date of birth; the employee's health care provider completes Part A (medical facts) and Part B (amount of leave needed) and signs. The employer completes Part B of the PFML-1 request.

Submitting and payment

How to submit it
The provider returns the completed PFML-7 to the employee, who sends it with the PFML-1 to The Standard by mail or fax.
Who pays
The packet sets no fee and names no payer for the provider's certification.

Good to know

  • Leave the diagnosis and primary ICD code blank for patients who work in Connecticut, Delaware, Massachusetts, Minnesota or Washington, as the form instructs.
  • The employee may sign Form PFML-6, an authorization letting The Standard contact the provider to clarify or authenticate the certification; the provider keeps it.
  • Part B asks for continuous incapacity dates, follow-up treatment or reduced-schedule needs, and flare-up frequency and duration over the next 6 months.
  • The same packet is published for CO, CT, DE, ME, MA, MN, OR and WA. State-plan claims use each state's own forms instead.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

What is Form PFML-6 in The Standard's packet?

An optional authorization the employee signs so The Standard can contact the provider to clarify or authenticate a completed certification. The employee gives it to the provider, who keeps it. Signing is not required, but it can shorten the time needed to clarify the form later.

Should the diagnosis go on The Standard's PFML-7?

Not for patients working in Connecticut, Delaware, Massachusetts, Minnesota or Washington; the form asks providers to omit the diagnosis and primary ICD code for those states. Elsewhere, the diagnosis and ICD code fields are completed with the other medical facts.

How quickly does The Standard decide a PFML claim?

The packet's instructions say The Standard accepts or denies the claim within 5 days once a complete claim is received. A complete claim means the PFML-1 with the employer's part and the provider's PFML-7, so an unanswered question can delay the decision.
