The Standard PFML-4: Family Member Care Certification

An employee covered by a private paid family and medical leave plan insured by The Standard who needs leave to care for a family member receives a Care of Family Member packet, and Form PFML-4 is its provider certification. The family member's provider documents the medical facts and the amount of care needed, and The Standard decides the claim.

Who completes it

The employee fills in the employee and family member information at the top; the family member's health care provider completes Part A (medical facts) and Part B (amount of care needed) and signs. The employer completes Part B of the separate PFML-1.

Submitting and payment

How to submit it
The provider returns the completed PFML-4 to the employee, who sends it with the rest of the packet to The Standard by mail or fax.
Who pays
The packet sets no fee and names no payer for the provider's certification.

Good to know

  • Do not enter the diagnosis or ICD code for patients who work in Connecticut, Delaware, Massachusetts, Minnesota or Washington; the form says this keeps it compliant with those states' rules.
  • The family member first signs Form PFML-3 releasing health information; the provider keeps PFML-3 and does not send it to The Standard.
  • Part B covers continuous incapacity, follow-up treatments, intermittent or reduced-schedule care, and flare-ups estimated over the next 6 months.
  • The Standard publishes the packet for CO, CT, DE, ME, MA, MN, OR and WA, and decides a complete claim within 5 days of receiving it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Should the diagnosis go on The Standard's PFML-4?

It depends on where the employee works. The form says not to include the diagnosis or primary ICD code for patients working in Connecticut, Delaware, Massachusetts, Minnesota or Washington. In other states, the diagnosis field and optional ICD code can be completed.

Who keeps the PFML-3 release with The Standard's packet?

The family member's health care provider. The family member, or an authorized representative, completes PFML-3 and gives it to their provider with the PFML-4. The provider retains it, and the employee keeps a copy for their records.

Does PFML-4 ask about military service?

Yes. Part A asks whether the family member is an active service member and, if so, whether the condition results from military service. It also asks about pregnancy, delivery dates and complications when the family member's condition is pregnancy.
