Principal GP62713 Covered Service Member Certification

Principal Life's GP62713 packet handles paid family leave to care for a covered service member, and qualifying exigency leave, under Principal's private plans in Colorado, Connecticut and Massachusetts. A one-page Health Care Provider Medical Certification documents the service member's condition, whether it is connected to military service, and how many days a week care is needed.

Who completes it

The employee completes the Employee Statement and the employer the Employer Statement; for covered service member leave, the service member's health care provider completes and signs the medical certification on page 6. Exigency leave needs no provider page.

Submitting and payment

How to submit it
The provider emails or faxes the certification to Principal's Group Life and Disability Claims unit, as the form instructs; the rest of the packet can also be filed online.
Who pays
The form states the patient is responsible for obtaining a complete form without expense to Principal, so Principal does not pay for it.

Good to know

  • The certification asks whether the patient requires care by this employee and whether the serious health condition is connected to military service.
  • Care frequency is given in days per week, along with the date the employee begins caring and the date care should no longer be needed.
  • Qualifying exigency claims instead require documents such as active-duty orders or a letter of impending activation, so no clinician is involved.
  • The signature line is marked 'No Stamp', so a stamped signature is not accepted. A Spanish edition, SP1964, is cited in the footer of the 06/2026 revision.

Frequently asked questions

Who pays for completing Principal's GP62713 certification?

Not Principal. The provider page states that the patient is responsible for obtaining a complete form without expense to Principal. Any charge for completing the certification is therefore between the practice and the patient or employee.

Does the provider complete GP62713 for qualifying exigency leave?

No. Exigency leave relates to a family member's active duty and is supported by documents like active-duty orders or a commanding officer's letter of impending activation. Only covered service member leave needs the provider's medical certification on page 6.

Which states does Principal's GP62713 apply to?

Principal uses this packet for paid family leave military caregiver claims in Colorado, Connecticut and Massachusetts under its private or equivalent plans. The same packet's employee and employer statements also serve qualifying exigency claims, which need no provider certification.
