Principal GP62712 PFML Provider Medical Certification

Principal Life uses the GP62712 packet for state paid family and medical leave claims under its private or equivalent plans when the reason is a serious health condition, the employee's own or a family member's. Pages 6 and 7 are the Health Care Provider Medical Certification: ICD-10 codes, onset, condition category, leave frequency and visit history. Principal decides the claim from the full packet.

Who completes it

The employee completes the Employee Statement (page 2) and the employer the Employer Statement (pages 3 to 5); each treating health care provider completes and signs the Medical Certification on pages 6 and 7.

Submitting and payment

How to submit it
The packet goes to Principal's Group Life and Disability Claims unit by mail, fax or email, or the claim can be filed online through Principal's claim website.
Who pays
The packet sets no fee and names no payer for the provider's medical certification.

Good to know

  • Principal wants a statement from every provider the patient sees for the claim; extra copies come from the employer's benefits administrator or Principal's claim website.
  • The signature line is marked 'No Stamp', so a signature stamp will not do; the physician's name, degree, specialty and phone are required.
  • The provider marks whether the patient can endorse and direct the use of benefit payments, and for the employee's own condition lists job duties they cannot perform.
  • A Spanish edition, SP1963, is available. Bonding claims use GP62711, which accepts a provider statement of the birth date instead of a form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Which conditions qualify on Principal's GP62712 certification?

The provider checks one or more categories: inpatient care (including a neonatal intensive care unit), a permanent or long-term condition under supervision, multiple treatments, a chronic condition with at least two visits a year, pregnancy or prenatal care, or incapacity with treatment.

Is Principal's PFML packet available in Spanish?

Yes. The footer of GP62712 cites the Spanish edition SP1963, which Principal posts alongside the English packet. The provider pages ask the same questions in both editions, including ICD-10 codes, onset date and visit frequency.

Can the GP62712 claim be filed online?

Yes. Principal's instructions say claims can be submitted on the web through its claim filing site, and employers can complete their statement there too. Paper packets can also be mailed, faxed or emailed to Principal's Group Life and Disability Claims unit.
