DD Form 2792: EFMP Family Member Medical Summary

The Department of Defense uses DD 2792 to document a military family member's special medical, developmental or behavioral health needs, both to enroll them in the Exceptional Family Member Program (EFMP) and to screen them before an overseas or remote assignment. The family member's own medical provider, military or civilian, completes the medical summary sections.

Who completes it

The sponsor provides the demographic information at the start of the form. The family member's medical provider, at a military treatment facility or in the civilian TRICARE network, completes the medical summary sections that follow.

Submitting and payment

How to submit it
The provider returns the completed form to the sponsor or family, who submit it to the installation EFMP office or the Family Member Travel Screening office.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who completes the DD Form 2792?

The family member's medical provider completes the medical summary sections, whether that provider works in a military treatment facility or is a civilian TRICARE network provider. The sponsor supplies the demographic information first, and the family submits the finished form to the EFMP or travel screening office.

Why does the DD 2792 show 'Please wait' instead of the form?

DD 2792 is built as an encrypted XFA form, which most browsers and non-Adobe PDF viewers cannot display. Download it from the Washington Headquarters Services forms page and open it in Adobe Reader to see and fill in the fields.

What is the DD 2792 used for?

It identifies family members with special medical, developmental or behavioral health needs for enrollment in the Exceptional Family Member Program, and it is used to screen family members before an overseas or remote assignment so the military can consider whether needed care is available there.
