EE-30 Transplant Authorization Request (EEOICPA)

When an energy employee with an accepted condition needs an organ or tissue transplant, the transplant provider asks DEEOIC for prior authorization on this form. It identifies the referring treating physician, the transplant facility and type, the organ transplant coordinator, and diagnosis and procedure codes. Three attachments are required: the treating physician's letter of medical necessity, initial and recent clinical evaluations, and the treatment protocol.

Who completes it

The transplant provider completes the form. Part D names the treating (referring) physician, whose letter of medical necessity describing the need for the transplant is a required attachment in Part G.

Submitting and payment

How to submit it
Submit it through the OWCP Web Bill Processing Portal; fax only if necessary, with the claimant ID on each page.
Who pays
No fee applies to the form; it determines whether DEEOIC covers the transplant for the accepted condition.

Good to know

  • Part B can list the claimant's authorized representative and phone number, a field that EE-22, EE-24, EE-26 and EE-32 do not carry.
  • Part F requires the transplant facility with its address and phone, the transplant type, and the organ transplant coordinator's name and phone.
  • The clinical evaluation attachment covers initial and recent workups, such as diagnostic studies and laboratory tests, alongside a copy of the treatment protocol.
  • Revised February 2026 under OMB 1240-0060; general procedures and drugs use Form EE-22 instead.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Which diagnosis codes go on EE-30?

Up to four ICD diagnosis codes in item E1, using ICD-9 for services before October 2015 and ICD-10 after. Each service line points to those diagnoses and gives from and to dates with a CPT or HCPCS procedure code.
