EE-24 Durable Medical Equipment Authorization (DEEOIC)

Suppliers request DEEOIC prior authorization for durable medical equipment on this form when an energy employee needs equipment for an accepted condition. Each service line gives a CPT or HCPCS code, units, a rental or purchase modifier, cost and, for rentals, duration. The treating physician's prescription and letter of medical necessity are required attachments, so the clinic's documentation underpins the request even though the supplier files it.

Who completes it

The DME provider or supplier completes the form. The treating physician supplies the prescription and letter of medical necessity required in Part E, and the supplier adds how the equipment meets that prescription.

Submitting and payment

How to submit it
The supplier submits it through the OWCP Web Bill Processing Portal; fax is for when necessary, with the claimant ID on each page.
Who pays
No fee is attached to the physician's prescription or letter; the request decides whether DEEOIC covers the equipment the supplier provides.

Good to know

  • Rental or purchase modifiers are RR (rental), NU (purchased new), UE (purchased used) and BO (oral enteral formula); rentals also need a duration such as 2 months.
  • List up to four ICD diagnosis codes and link each service line to them; ICD-10 applies to services after October 1, 2015.
  • Missing supporting documents can delay processing or lead to denial, so the prescription and letter should travel with the request.
  • General procedures, outpatient services and drugs go on Form EE-22 instead. EE-24 was revised February 2026 under OMB 1240-0060.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who files Form EE-24, the doctor or the supplier?

Usually the DME supplier, as the rendering provider with an OWCP provider ID. The treating physician's role is the prescription and letter of medical necessity, which must be attached. DEEOIC warns that failing to include supporting documentation may delay processing or lead to denial.

What codes does Form EE-24 accept?

Service lines take CPT or HCPCS procedure codes with units, a rental or purchase modifier and total cost. Diagnosis codes go in D1, up to four, using ICD-9 only for dates of service before October 2015 and ICD-10 for later dates.
