Hawaii OCOCA Attending Provider Reporting Form

Hawaii attending providers file this four-page report when they prescribe medical aid-in-dying medication under the Our Care, Our Choice Act (HRS Chapter 327L). It records patient demographics and insurance, diagnosis and hospice status, the oral and written requests, compliance steps, referrals to the consulting and counseling providers, and the medication prescribed. Three companion forms are attached.

Who completes it

The attending provider, a Hawaii-licensed physician or APRN, completes and signs the whole form. The consulting provider, counseling provider and patient complete the attached forms.

Submitting and payment

How to submit it
Mail it with all supporting documents to the Hawaii Department of Health, Office of Planning, Policy, and Program Development, within 30 calendar days of the prescription date; do not fax or email patient information.

  • Provide the Final Attestation Form at the second oral request and advise the patient to complete it 48 hours before self-ingesting.
  • If the patient is not enrolled in hospice, the form prompts the provider to recommend enrollment and record that it was recommended.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
