Hawaii OCOCA Attending Provider's Follow-Up Form

The last of Hawaii's six Our Care, Our Choice Act forms closes out a medical aid-in-dying case. The attending provider reports the date of death and underlying illness, whether the patient died from ingesting the medication, any complications or barriers, and whether the patient was enrolled in hospice. The patient's Final Attestation Form can be attached.

Who completes it

The attending provider, a Hawaii-licensed physician or APRN qualified to make the diagnosis and prognosis, completes and signs the whole form.

Submitting and payment

How to submit it
Mail a copy to the Hawaii Department of Health, Office of Planning, Policy, and Program Development, within 30 calendar days after notification of the patient's death; do not fax or email patient information to DOH.

Good to know

  • The 30-day clock runs from notification of the qualified patient's death, whether from the prescribed medication or any other cause.
  • Ingestion, complications and hospice enrollment each have an Unknown option, and the date of death can also be marked unknown.
  • The provider attests to licensure under HRS Chapter 453 or 457 and that all Our Care, Our Choice Act requirements have been met.
  • Forms effective July 1, 2024 reflect 2023 changes that let APRNs act as attending or consulting providers.
