WSCC Psychologist Progress Report (CS102): NWT, NU

Psychologists treating a WSCC claimant in the Northwest Territories or Nunavut report ongoing treatment on this two-page form after the Psychiatric/Psychological Initial Report (CS103). Page 1 covers sessions completed, up to five treatment goals ranked by priority, modalities and progress since the last report. Page 2 addresses return-to-work readiness and psychological barriers, and how many more sessions are requested with the proposed plan and session frequency.

Who completes it

The treating psychologist completes and signs the whole report, printing their name, registration and licence number. The worker and employer do not complete any section.

Submitting and payment

How to submit it
Email or fax it to WSCC's NWT or Nunavut claims department using the contacts on the form; WSCC's invoicing FAQ asks for medical reports within three business days of attending the worker.
Who pays
Bill sessions separately on the WSCC Medical Invoice form, sent to Financial Services within one year of the service, because the WSCC wants reports and invoices sent apart.

Good to know

  • If the worker is not ready to return to work, the form asks you to name the psychological barriers; N/A is also an option.
  • Requests for further treatment go on page 2: tick yes, state how many additional sessions you want, and describe proposed modalities and session frequency.
  • The form asks whether factors such as a pre-existing condition may complicate recovery, with an explanation if yes, and for the date of the next visit.
  • Dates are written month, day, year; the printed revision is CS102 0924, and the French version is titled Rapport d'évolution du psychologue.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

How do I request more psychology sessions from the WSCC?

Use page 2 of the Psychologist Progress Report. Answer whether the worker requires further psychological treatment, give the number of additional sessions requested, and describe the treatment plan, including proposed modalities and session frequency, along with the date of the next visit. Progress since your last report on page 1 supports the request.

What is the difference between WSCC forms CS102 and CS103?

CS103, the Psychiatric/Psychological Initial Report, documents the presenting problem, history, diagnosis or clinical formulation, test results and initial treatment plan. CS102, the Psychologist Progress Report, follows it, reporting sessions completed, ranked treatment goals, interventions, progress since the last report and any request for further sessions.

Is the WSCC Psychologist Progress Report available in French?

Yes. WSCC publishes CS102 as a fillable PDF in English and in French, as Rapport d'évolution du psychologue, and the English form notes that a French version is available. The same form serves workers in both the Northwest Territories and Nunavut.
