WorkSafeNB Physiotherapy Progress Report

New Brunswick physiotherapists use this two-page report to update the WorkSafeNB case manager on a worker's treatment. It moves from subjective symptoms through objective findings such as posture, neurological status, range of motion, muscle and stability testing, to a problem list, clinical impression, progress toward functional goals, barriers to rehabilitation, return-to-work comments and the current treatment plan.

Who completes it

The treating physiotherapist completes and signs it, naming the treatment facility, referring physician and case manager. Discharge date and disposition are recorded on the same form when treatment ends.

Submitting and payment

How to submit it
Forward it to WorkSafeNB by fax or mail, or upload it through MyServices; if you fax a form, don't mail it as well.
Who pays
WorkSafeNB pays for treatment covered under the Workers' Compensation Act, and its agreement with New Brunswick Private Practitioners of Physiotherapy sets standards for timely reports.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who receives the WorkSafeNB physiotherapy progress report?

WorkSafeNB, for the case manager named on the form. The report notes that section 41(10) of the Workers' Compensation Act authorizes you to release the information, and that anyone with a direct interest in a claim under review may examine the document.

Is there a separate WorkSafeNB physiotherapy discharge form?

The progress report itself has discharge date and discharge disposition fields at the end of page 2, after comments and recommendations. WorkSafeNB's health care forms page lists only this report for physiotherapists, and it notes that occupational therapy forms are under review.

Is the physiotherapy progress report available in French?

Yes. WorkSafeNB posts a French version, Rapport d'évolution en physiothérapie, on its French-language site, travailsecuritairenb.ca. The English version is the only form listed for physiotherapists on WorkSafeNB's health care forms page, and it is not a fillable PDF.
