WorkSafeNB Estimate of Required Dental Services

After a work-related dental injury in New Brunswick, the attending dentist or denturist completes this one-page estimate and forwards it promptly to WorkSafeNB. It records the date of incident, a tooth chart of teeth damaged by the incident and teeth already missing, a detailed description of the damage, and itemized procedure codes with laboratory charges and professional fees.

Who completes it

The attending dentist or denturist completes, stamps and signs the form, including the worker's details and Medicare number and the employer's name and address. The worker does not sign it.

Submitting and payment

How to submit it
Forward it promptly to WorkSafeNB by fax or mail, or through MyServices, and print clearly if completing it by hand; if you fax it, don't mail it as well.
Who pays
WorkSafeNB pays for covered treatment of the injured worker, and the estimate is priced with procedure codes from WorkSafeNB's Dental Fee Guide, with a payee code box for the provider.

Good to know

  • Mark each tooth damaged by the workplace incident with the letter A and each tooth missing before the incident with the letter M.
  • List the laboratory charge and the dentist or denturist fee separately for each procedure code and tooth number, with a total charge per line.
  • Describe the dental damage caused by the incident in detail in the free-text box before the itemized estimate; an additional comments box follows it.
  • The form is printed as revision 07/19, and WorkSafeNB's forms list dates it June 27, 2019.
