Health PEI Imaging Requisition (Q74-05)

Health PEI's provincial Diagnostic Imaging departments use one requisition for general radiography, CT, nuclear medicine, bone mineral density, mammography, ultrasound and MRI. The physician or nurse practitioner sets the priority, describes the exam and area of interest, and gives the provisional diagnosis, clinical history and relevant previous imaging. MRI requests also need the safety questions on the reverse.

Who completes it

The ordering physician or nurse practitioner completes the requisition and signs it, with office phone and fax numbers and any extra report recipient. Technologists and radiologists complete the department sections at the time of the exam.

Submitting and payment

How to submit it
Health PEI asks that urgent and semi-urgent x-ray requisitions be faxed to the QEH Diagnostic Imaging department, which books the patient within 24 to 48 hours. For routine exams, the patient takes the requisition and books by phone or online.

Good to know

  • The MRI side asks for weight, metal injury to the eyes, claustrophobia, sedation, pregnancy, kidney disease and implanted devices; an incomplete MRI requisition is returned.
  • Sedation for MRI must be prescribed by the referring physician, because the MRI department does not supply medication.
  • STAT x-rays are only for suspected fracture from an injury under 3 weeks old, chest pain or acute shortness of breath, or suspected cancer or metastasis.
  • Echocardiograms use their own Echocardiogram Request Form, and older versions of this requisition should be destroyed.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How do I send an urgent x-ray request in PEI?

Health PEI asks clinicians to fax urgent and semi-urgent x-ray requisitions to the QEH Diagnostic Imaging department; reception books the appointment and contacts the patient within 24 to 48 hours. Patients may go straight to the department for STAT x-rays only if they meet the STAT criteria, and the requisition must be marked STAT.

What is the current version of the Health PEI imaging requisition?

The form printed Q74-05 (01/21), with the identification code 101074-0005 (01/21). Health PEI revised it in 2021, adding booking numbers for every site, moving the malignancy question under clinical history, and adding DND and non-Canadian resident to billing. It asked sites to destroy older versions.
