Manitoba MPI Subsequent Therapy Report (Physio, AT)

Physiotherapists and athletic therapists use this one-page follow-up report to tell Manitoba Public Insurance (MPI) how a PIPP claimant is responding to treatment. It asks whether the patient is improving, which collision-related symptoms remain, current signs and outcome scores, diagnoses, injury category and work status, and summarizes progress towards treatment goals.

Who completes it

The treating physiotherapist or athletic therapist completes and signs it with the MPI registered account number. The patient or guardian signs the authorization that releases the report to MPI in support of the claim.

Submitting and payment

How to submit it
MPI accepts it by mail, fax or email to its reports inbox; for email, put only the claimant's initial and claim number in the subject line.
Who pays
Treatment is paid at MPI's physiotherapy and athletic therapy service-agreement rates, and MPI asks for the claim number, claimant name, specific service and MPI Registered Account Number on every invoice so it can pay promptly.

Good to know

  • At least two of seven listed outcome measures must be checked and scored: NPRS, Roland Morris, Neck Disability Index, Yellow Flags, LEFS, DASH and SF-12.
  • The management plan asks whether a referral has been made to another health care practitioner and, if so, where.
  • Work status questions ask whether returning to work would adversely affect the condition, preclude travel to work, prevent required tasks or pose a safety risk.
  • MPI's physiotherapy guidelines expect key treatment decisions within the first ten treatments or two to three weeks, whichever comes first, including triage to complex or multidisciplinary care.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Is the MPI Subsequent Therapy Report available in French?

Yes. MPI's physiotherapy provider page links a French version of the Subsequent Therapy Report as a Word document, alongside the English PDF. The Therapy Discharge Report, by contrast, has no French version listed on that page.

What does MPI ask about driving on the Subsequent Therapy Report?

The form asks whether the patient's condition poses a safety risk to operating a motor vehicle, with yes or no boxes next to the clinical diagnoses and injury category. The same question appears on MPI's initial and discharge therapy reports.
