Employers and beneficiaries use this three-page form to claim group life insurance from iA Financial Group after the death of a member, spouse or dependent child. The employer completes the first page and the beneficiary the second, while page 3 holds a short Physician's Statement on the cause of death. That statement is mandatory only for larger coverage amounts; otherwise an official death certificate may be enough.
The physician completes and signs the Physician's Statement on page 3, covering the principal and contributing causes of death, the date of onset and the treatment period. The employer completes the Employer's Statement, and the beneficiary (claimant) completes the Beneficiary's Statement and signs the confirmation and authorization.

Upload the fillable PDF once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

The claim instructions say the Physician's Statement must be completed and signed when the optional life insurance amount, or the combined basic and optional amount, exceeds the thresholds printed on the form. In those cases the death certificate and the Physician's Statement are both required; otherwise the beneficiary can provide either one.
It asks for the deceased's full name, smoker or non-smoker status, dates of death and birth, place of death, the principal cause of death, any causes that contributed to death, the date of onset of the illness or event, and the dates you treated the deceased, followed by your name, signature and address.
Yes. The English form F54-361A, edition 25-02, is a fillable PDF, and iA publishes a French version numbered F54-361. Both are on iA's group claims page for critical illness, death and accidental dismemberment, which asks claimants to print the form, sign it by hand and send it with required documents.
