Canada Life M7532: Nursing Care Health Assessment Form

Canada Life requires a pre-care assessment to set how much private nursing care a group benefit plan will cover, and this form should be submitted before nursing care begins. After review, Canada Life contacts the provincial home care case coordinator and tells the claimant in writing what coverage, if any, applies and the number of eligible hours.

Who completes it

The patient's physician completes Part 2 (current medical information), printing clearly. The plan member completes Part 1, the provincial home care case coordinator or manager completes Part 3, and the plan member and patient sign Part 5.

Submitting and payment

How to submit it
The form is mailed to the Nursing Specialist in Canada Life's Medical and Dental Claims Management; urgent requests can be faxed or emailed, though email is not a secure medium.
Who pays
The form states that fees for providing medical information are not payable by the plan.

  • Recommend RN, RPN/LPN or HCA/PSW care; for RN or RPN/LPN, list specific nursing treatments with frequency and length, not duties an HCA or PSW could do.
  • The patient must also apply to the provincial home care program and authorize it to exchange information with Canada Life.
  • Part 2 includes a body-systems checklist, vital signs, medications with route, dose and frequency, an MMSE score, adaptive equipment and help needed with daily living activities.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When should Canada Life's Nursing Care Health Assessment Form be submitted?

Before nursing care begins. Canada Life requires a pre-care assessment to establish the amount of nursing care covered under the group plan, and it asks that the form be completed in full and sent before care starts. Incomplete forms delay the assessment.

What level of nursing care can the physician recommend on M7532?

Part 2 offers RN, RPN/LPN and HCA/PSW levels, noting coverage is based on plan design. For RN or RPN/LPN care, the physician must specify the nursing treatments, such as dressings or injections, with frequency and length, and these cannot be duties an HCA or PSW could perform.

Who pays for completing the Canada Life nursing care assessment?

Not the plan. The completion instructions state that fees for providing medical information are not payable by the plan, so any physician fee for Part 2 is not reimbursed under the group benefit plan.
